Prince William Hockey Club

Congratulations on being selected to a PWHC Travel Team.  To accept your position on the team, a complete packet must be turned in to your coach during or before the Team Meeting (between 7-10pm at Prince William County Complex - exact times for each team to be posted soon) on Monday, May 16th:

• Travel Registration and Commitment Form
• 2011-2012 USA Hockey Registration Number MUST be provided on registration form - visit www.usahockeyregistration.com to register for 2011-2012
• USA Hockey Code of Conduct Form
• Consent to Treat / Medical Form
• Deposit of $350 - check or credit card (see registration form)
• Uniform Order Form (please review as there are new requirements this year)
• Copy of your player's Birth Certificate
• IF you were not a 2010-2011 PWHC Travel player, you must also submit a Number Selection Form with your packet

IMPORTANT: A completed Registration Packet with deposit must be turned in by the Team Meeting on May 16th, to secure your player's position.  If a completed packet and deposit are not turned in by then, the player's position will be forfeited.

NEW FOR 2011-2012 - ALL PWHC TRAVEL PLAYERS ARE REQUIRED TO HAVE THE SAME STYLE JERSEY AND SOCKS

If your jersey and socks are of an older style, you will need to order the current style (see photo) at this time.  Complete the attached Uniform Order Form and return with your registration paperwork.  Payment will not be due until items are delivered. It is important to get your order in along with your registration packet to ensure delivery in plenty of time. 

The vendor has assured PWHC that this style will remain available indefinitely. That is why we are finally able to achieve a consistent look throughout all of our teams.
